Wow, you're lucky that didn't come flying through the bellhousing!
I can't come up with any explanation for why only the outside of the disc appears to be touching the flywheel, other than just a faulty disc.

with the sounds it made, im surprised it didnt come through the bell! i wasnt abusing it at the time of the boom, but i did dyno it 6 hours earlier lol

The fact that it failed at a low rpm is what saved your feet. The dyno shouldn't be hard on the clutch unless it was slipping.
